Long-Term Disability Overview
MetLife’s Long-Term Disability (LTD) insurance provides income protection if you’re unable to work for an extended period due to a serious illness or accidental injury. This benefit helps replace a portion of your income so you can continue to meet financial obligations during your recovery.

Monthly Benefit: Replaces 60% of your monthly earnings, up to $2,500/month

Important
If you previously declined this coverage and are now enrolling, you must complete and submit a Statement of Health (SOH) form to HR by December 8, 2025. Download the form here: 2026 Statement of Health Form
Note: You only need to complete one Statement of Health (SOH) form per person. The same form can be used for Voluntary Life Insurance, Short-Term Disability, and Long-Term Disability. There is no need to submit separate forms for the same individual.
