Plan Highlights

  • Monthly Benefit: Replaces 60% of your monthly earnings, up to $2,500/month

  • Benefit Duration: Payments continue up to age 65 (if eligible)

  • Waiting Period: Benefits begin after 90 consecutive days of disability

Additional Features

  • Support services to help you return to work

  • Financial incentives for participating in return-to-work programs

  • Assistance with applying for Social Security Disability benefits

Important

If you previously declined this coverage and are now enrolling, you must complete and submit a Statement of Health (SOH) form to HR by December 8, 2025. Download the form here: 2026 Statement of Health Form

Note: You only need to complete one Statement of Health (SOH) form per person. The same form can be used for Voluntary Life Insurance, Short-Term Disability, and Long-Term Disability. There is no need to submit separate forms for the same individual.
