Supplemental Life Insurance Overview

Jaynes offers Supplemental Life Insurance through MetLife for employees who want added financial protection beyond the company-paid Basic Life Insurance. This voluntary coverage allows you to increase your life insurance benefit to better support your loved ones in the event of your passing.

You may want to consider this policy if your employer-paid life insurance policy isn’t enough to support those who rely on your income, or if you want additional coverage for specific costs, such as burial fees. This life insurance option is voluntary, and premiums will be deducted from each paycheck.

Guaranteed Issue Amounts*

Employee Coverage: up to $100,000
Spouse Coverage: up to $25,000
Child Coverage: up to $10,000

*Guaranteed issue amounts are only available to newly hired employees or those newly eligible for this benefit. They do not apply to employees who previously declined coverage.

Statement of Health (SOH) Requirement

You must complete and submit a Statement of Health (SOH) form to HR by December 8, 2025, if any of the following apply:

You are a new employee (or newly eligible for this benefit) and want coverage above the guaranteed issue amount
You previously declined coverage and are now enrolling yourself, your spouse, or a dependent child (regardless of the amount selected)
You already have coverage but want to increase it beyond the allowed annual limits:

Employee: More than $10,000/year
Spouse: More than $5,000/year
Child: More than $1,000/year

Note: You only need to complete one Statement of Health (SOH) form per person. The same form can be used for Voluntary Life Insurance, Short-Term Disability, and Long-Term Disability. There is no need to submit separate forms for the same individual.
