To qualify for the discounted non-tobacco rate, you (and your spouse, if enrolled) must complete a tobacco/nicotine test at the Jaynes Healthcare Clinic by
December 8, 2025. Testing can be done during your biometric screening.

You can also switch providers at any time. Imagine360 administers the plan and serves as your primary contact for benefits, eligibility, claims, and billing support.
